ANGELA DAWN FORTH, 42, of Huntington, W.Va., died Sunday, June 10, 2018 at St. Mary's Medical Center.  She was born May 9, 1976 in Huntington.  Her father, Gazle Puskas Jr. preceded her in death.  She leaves behind her loving husband, Russell Forth; her mother Virginia Rakes Puskas of Kenova, W.Va.; three beloved children, Brandon Forth, Nikki Forth, and Wendi Forth, all of Huntington; two brothers, David Puskas of Rush, Ky., and Robert (Belinda) Puskas of Mooresville, N.C.; and aunts, uncles, nieces, nephews, cousins, and friends too numerous to mention.  Funeral services will be conducted 2 p.m. Tuesday, June 12, 2018 at Johnson Tiller Funeral Home.  Burial will follow at Community Memorial Gardens.  Friends may call two hours prior to service on Tuesday.
